What Is Carpal Tunnel Syndrome — And Why Does It Persist?

The carpal tunnel is a narrow passageway in the wrist formed by the carpal bones and the transverse carpal ligament. The median nerve — which provides sensation to the thumb, index, middle, and half of the ring finger — passes through this tunnel along with nine flexor tendons. When the tunnel narrows or its contents swell, the median nerve is compressed, producing the characteristic burning, tingling, numbness, and weakness of carpal tunnel syndrome.

What most patients are not told is that carpal tunnel syndrome frequently has contributors far beyond the wrist itself. The median nerve originates from the C6, C7, and C8 nerve roots in the cervical spine. Compression anywhere along its path — from the cervical spine through the thoracic outlet, across the pronator teres in the forearm, and into the carpal tunnel — can produce identical symptoms.
